Als Kompromiss aus beiden unterschiedlichen Verzeichnissen wurde ein einheitliches Verzeichnis mit 139 Krankheitsgruppen beschlossen und 1864 herausgegeben. Eingeteilt wurden die Krankheiten nach Lokalisation. In den darauffolgenden Jahren wurden fortlaufende Optimierungen des Systems vorgenommen. Aus diesen ging schließlich die ICD-1-Klassifikation hervor, die im Jahre 1893 vom International Institute of Statistics angenommen wurde. Als die Weltgesundheitsorganisation ins Leben gerufen wurde, übernahm diese die Verantwortung für die Validität des Verzeichnisses. Durch die 6. Revision wurde die ICD-6 der WHO entwickelt. Ab diesem Zeitpunkt an wird das Ordnungssystem unter der Leitung der WHO revidiert und ergänzt. Die derzeit neueste Fassung wird als ICD-10 bezeichnet.
